<ns1:description language="de">Masterarbeit, Fachhochschule St. Pölten, Masterstudiengang Soziale Arbeit, 2020</ns1:description>
<ns1:description language="en">Caring for relatives or family members can cause severe stress that requires different
forms of support and help. In the area of ambulant care and nursing for the elderly
medicine and nursing are dominant professions while the profession of social work is not
represented so far. Knowing the needs of informal caregivers and contributing its
methods and knowledge about complex social situations social work could be an
invaluable partner in this field. In 2011 two students from the University of Applied
Sciences in St. Pölten developed a model for implementing social diagnostics in
ambulant care and nursing for the elderly. The model has not been tested in practice yet.
The aim of this thesis is to test and evaluate the model in the field. The model consists
of an assessment of a social situation of an informal caregiver by a mobile care worker
and a counseling by a social worker. After some theoretical improvements the model
was tested during a period of three months in cooperation with Volkshilfe
Niederösterreich and nine nurse managers. To evaluate and improve the model we
analyzed the questionnaires and the counseling sessions. Additionally we organized
interviews and a group discussion with the nurse managers.
We focus on three different key aspects. The first part of this thesis deals with the
improvement of the questionnaire and its contents for further practice. The second part
focuses on the realization of the assessment by the nurse managers. Through the
experience and feedback of nurse managers a guideline with practical recommendations
and suggestions was developed. The third part deals with the social work counseling
and develops a concept including possible intervention for future social workers in the
field of ambulant care and nursing for the elderly. The results of all key aspects are
concentrated in a manual for nurse managers and social workers. </ns1:description>
