Wiener Gemeindebezirk erfolgen? Themenbereiche: 1. Zielgruppe(n), 2. Soziales Rezept, 3. Link Worker:in, 4.

<lom:langstring xml:lang="de">Current Situation: Since the 1990s social prescribing has been implemented in Great Britain's health care system. This concept takes social determinants into account in ad-dition to medical aspects of health. Clients get referred to creative or sport activities or counseling services. Within the reorientation of primary care, this should also be increas-ingly applied in Austra in the sense of the biopsychosocial model. The roots of this un-derstanding of health can already be found in the work of Ilse Arlt, a pioneer of social work, at the beginning of the 20th century. Research question: How can the practical implementation of social prescribing take place in the Dr. Ouhadi - Dr.in Pilz group practice for general medicine (stage 1) and subsequently in the PVE (stage 2) in the 15th district of Vienna? Topics: 1. target group(s), 2. social prescription, 3. link worker, 4. interface management and 5. co-deter-mination, re-utilisation of service, discontinuation and evaluation Aim: Develop specific recommendations for implementing social prescribing in practice and testing them during a two-month implementation phase Methods: Method triangulation, consisting of needs assessment, document analysis, questionnaire and expert interview Selected Results: ◼ Difficulties in understanding and structural conditions prove to be an impediment to reach certain target groups. ◼ The client-centered approach of referrals is beneficial for the process of social pre-scribing. ◼ Beside referrals the focus is on social work activities. Due to that fact social workers are particulary suited as link workers. ◼ Networking and regular exchange between the members oft the primary care facility and the 3rd sector are the basis for good cooperation. ◼ Evaluations, taking the client's point of view into account, contribute to the optimisa-tion of processes in the primary care facility.</lom:langstring>
